Job Summary:
This position is responsible for the creation of Person Centered Plans for individuals with disabilities across their authorized programs. In addition to planning, this role will be responsible for scheduling of clients across programs and supporting the staffing of the programs as designated by their supervisor.
Essential Functions:
  1. Leads each DSP on his/her specified case-load to ensure that PCP’s (Person Centered Plans) are structured for each client.
  2. Ensures that opportunities offered to clients respect the principles of dignity of risk, and person centeredness with a primary focus on community inclusive opportunities.
  3. Models and ensures that Direct Support Professionals provide quality support to clients throughout their service provision.
  4. Leads the work/provides guidance to all Direct Support Professionals and Vocational Rehabilitation Specialists.
  5. Leads and participates in regular staffing meetings to ensure that the needs of Skills Program, Social Program, and the Pre-Vocational Program are met and maintains a level of quality.
  6. Tracks accomplishments on client goals to report on a quarterly and annual basis for review for each client within the caseload.
  7. Tracks data and documents any time of concern or desire to change support as requested. Reflects into the development and implementation of the individuals person centered planning.
  8. Monitors authorizations for service through a state database to ensure approval prior to service delivery.
Other Duties:
Manages/administers assigned programs for adherence to all licensing and accreditation standards. 
Develops/implements meaningful classes and activities for DSP’s to utilize with clients.
Provides scheduling to assigned areas to ensure continuity of services.
Provides support by providing coverage for DSP’s or coaching DSP’s through an issue for documentation of services.
Developmental Skills:
  1. Participates in ongoing training, webinars, and meetings assigned to ensure professional growth and adherence to changing regulatory standards as applicable to service lines in their specified area.
  2. Remains informed of local community opportunities and supports to enhance individuals served within their community through services or referral.
  3. Meets Department of Behavioral Health and Developmental Services Supervisory Competencies.
  4. Participates in regulatory review teams to ensure licensing and accreditation standards are met.
  1. In collaboration with the Departmental Manager, supports the establishment and maintenance of partnerships within the community to develop opportunities for individuals served.
  2. Communicates thoroughly with all components of an individual's treatment team to ensure quality service implementation.
  3. Collaborates with other Lead Direct Support Professionals to ensure therapeutic referral and transition across services provided.
  4. Establishes rapport across departments to increase support of client opportunity throughout services.
  5. Reports routinely to direct supervisor areas of risk, incident reporting, and stakeholder feedback to address and enhance NW Works service delivery standards.
  6. Ensures that the documentation per client is accurate and completed within NW Works established timeline.

Required Qualifications/Education:
  1. Bachelor's Degree in an approved Human Services field or commensurate experience of ten years.
  2. Required 2 years of verifiable work experience working with individuals with developmental disabilities.
  3. Verification of a clear driving record.
  4. Ability to drive a 12 passenger van.

  1. Proficient in Word, Excel, PowerPoint, and other applicable database programs.

  5. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
