Key Responsibilities

  • Handles initial contact with potential clients and families, guiding them through the application process. ?
  • Reviews applications and supplemental documentation to determine eligibility. ?
  • Maintains accurate documentation and records for all applicants. ?

Coordination Communication

  • Collaborates with case managers and service providers for client transitions. ?
  • Acts as a welcoming point of contact for inquiries from families and community organizations. ?
  • Schedules facility tours and provides support to prospective clients and families. ?

Data Entry Reporting

  • Responsible for entering client information into databases and generating reports. ?
  • Tracks admissions statistics and maintains databases related to inquiries and referrals. ?

Knowledge Skills Abilities

  • Requires strong organizational, communication, and analytical skills. ?
  • Knowledge of Medicaid Waiver Services and data collection processes is essential. ?

Minimum Education Certifications Experience

  • Preferred Bachelor’s Degree in Human Services or Business; High School Diploma or GED required. ?
  • At least 2 years of experience in a training or teaching environment. ?
